Guidance Paperwork
To make sure our clients get the most out of their one-on-one guidance experience we ask students and their parents to complete a brief questionaire about the student's interests, personality and academic history. This allows us to make sure that your student is matched with the guidance counselor best equipped to handle your student's unique needs. This section contains links to the forms you will be asked to complete before a guidance counselor is assigned to your family.
We work hard to select only the most qualified guidance counselors for our staff so we take each assignment very seriously. If you have any questions while completing these forms please contact us.
Survey for Students
Our student intake survey allows our guidance staff to get a better undrestanding of your academic abilities, interests and personality. This allows us to match you with a guidance counselor who is a good fit for you, as well as giving our staff some important insights into who you are as a student.
Be prepared to take at least 30 minutes to complete this form. It is important that you answer these questions honestly and take the time to explain any answers that you think need additional detail. You can then submit your form via email (by using the embedded link) or by printing and faxing or mailing the form to our office.
Survey for Parents
Our parent survey attempts to gain a better understanding of the student from the parent or guardians' perspective. This survey will take about 30 minutes to complete. If both parents or guardians are available please be sure to complete the form together. Please know that the survey is confidential and answers can be withheld from the student at your request.
